New Patient Registration

Please visit the surgery to collect the relevant documents for completion for each member of the family wishing to register. Forms can also be downloaded below.

Once the forms are completed return them to the surgery in person (with photograpic ID and proof of address), you will also be required to make a new patient check appointment with the nurse for everyone over 5yrs old.

Medical treatment is available from the date of registration. Please contact reception for further information.

Practice Boundaries

In order to register as a patient at Bentley Village Surgery, you must live within our practice boundaries:

  • To the north, A287 from Farnham to Odiham, excluding Odiham town centre. 
  • To the west, A32 including villages either side.
  • To the south from Fourmarks through Farringdon to B3004 from Alton to Kingsley including Oakhanger.
  • Then in a line east to include Binsted, Dockenfield, Rowledge and Wrecclesham.
